Working with clients with injury is way out of my scope of practice?

I would say no.

All of my clients have gotten the okay from their doctor, orthopedic specialist, physical therapist, and massage therapist to start a rotator cuff exercise program and have been encourage by their health care team to start a post rotator cuff injury exercise program.
